What Is Direct Primary Care?
Direct primary care (DPC) is a model for primary care where a patient or member pays a monthly fee for services which includes:
-Routine/annual primary care and health related screenings
-Personalized care, Relationship based care; Unrushed Visits, Continuity of Care, Patient-Autonomy
-Multiple ways to communicate with PCP include text and email, response by PCP within 24 hours of message received
-Follow up visits including health education, handouts, conversation about care planning
-Sick Visits offered without waiting and in the comfort of your home
-Discounts on labs, imaging and medications
-Community Events, physical and social engagements
-Monthly Newsletter
Membership is $90/month with a one time enrollment fee of $100

My Mission, My Calling…
My philosophy of care is trauma informed, holistic and lifestyle medicine that are centered on you as a person and not disease. These are the four petals of the sunflower logo for the practice I’ve created. Although I offer chronic disease management, I am an advocate to my patients that you are not defined by a disease and love to coach and encourage through lifestyle recommendations to spur on resilience, health and wellness. In addition to be being a board certified family nurse practitioner, I hold a graduate certified in public health, certificates in lifestyle medicine including nutrition, weight and wellness, and bachelors degrees in nursing, communication and international studies. Prior to become a nurse practitioner, I was a medical mission, ER nurse, forensic nurse and nursing instructor and volunteered with search and rescue teams as well as EMS in my communities. I am passionate about being a steward in my communities with the mission to increase access to authentic healthcare and offer free healthy community activities during the month including a walk in the park, book club and health fair. When I'm not serving or caring for others, you can find me enjoying a hike or at home with my two Maine coon cats (Perseverance and Bentley). It is my privilege to care for you and support you and appreciate you!

Not a member but need urgent care or visiting from out of town and feeling sick? Simple urgent care. telehealth, and office visits starting at $100/visit.
Examples of sick visits may include: Sore Throat, Earache, Viral Illness, Headache, Gastrointestinal Disease, Constipation, Diarrhea, Urinary Tract Infection, Minor Sprains, Muscle Pain, Tick Bite, Rashes.
Not a Member but need a Sports or Camp Physical, Work Form completed or other acute care needs- one time visit for non-members $100

Mending Insurance is accepted! However, other insurance plans do not reimburse for DPC but you can apply your monthly membership fee towards your deductible and use your HSA to pay your membership
Direct Primary Care is a Membership based care model. For $90/month patients get unrestricted care without insurance.
Insurance is still recommended for catastrophic and emergency care.
Insurance can be billed for labs and medications

A referral to specialty care such as cardiology, pulmonary, orthopedics and others may be needed to best direct your care. However, there are specialists who network with DPCs and your referral is personalized by Emily for continuity of your care.

House calls are for everyone! There is an add on charge for house calls of $50/visit within a 20 mile radius offered to members. Emily offers care to patients of all ages and a house call can be to your home, office or work environment or meeting at a location of your choice such as going for a hike or helping you to make smart choices at the grocery store. This model gives patients the time and control over their healthcare.

We will review your health goals and needs and determine if this is a good fit for your care.
Complete membership agreement and registration; this includes requesting records from your previous primary care.
Once records are received and reviewed we will schedule a visit to get to know each other further and establish your care.
You will be able to self schedule appointments that can be in person, in office, telehealth or mobile/house call. Same day appointments are available. Same day responses to email, phone and text are directly from Emily.
