What Is Direct Primary Care?

Direct primary care (DPC) is a model for primary care where a patient or member pays a monthly fee for services which includes:

-Routine/annual primary care and health related screenings

-Personalized care, Relationship based care; Unrushed Visits, Continuity of Care, Patient-Autonomy

-Multiple ways to communicate with PCP include text and email, response by PCP within 24 hours of message received

-Follow up visits including health education, handouts, conversation about care planning

-Sick Visits offered without waiting and in the comfort of your home

-Discounts on labs, imaging and medications

-Community Events, physical and social engagements

-Monthly Newsletter

My Mission, My Calling…

My philosophy of care is trauma informed, holistic and lifestyle medicine that are centered on you as a person and not disease. These are the four petals of the sunflower logo for the practice I’ve created. Although I offer chronic disease management, I am an advocate to my patients that you are not defined by a disease and love to coach and encourage through lifestyle recommendations to spur on resilience, health and wellness. In addition to be being a board certified family nurse practitioner, I hold a graduate certified in public health, certificates in lifestyle medicine including nutrition, weight and wellness, and bachelors degrees in nursing, communication and international studies. Prior to become a nurse practitioner, I was a medical mission, ER nurse, forensic nurse and nursing instructor and volunteered with search and rescue teams as well as EMS in my communities. I am passionate about being a steward in my communities with the mission to increase access to authentic healthcare and offer free healthy community activities during the month including a walk in the park, book club and health fair. When I'm not serving or caring for others, you can find me enjoying a hike or at home with my two Maine coon cats (Perseverance and Bentley). It is my privilege to care for you and support you and appreciate you!
